SMS HEALTH OFFICE

END OF YEAR MEDICATION REMINDER

Attention Parents:

Please pick up any unused medication(s) that your child has in the health office by the last day of school. Parents can come in anytime during the school day to pick up medications. The school cannot store medication in the office over the summer and unclaimed medications will be destroyed.

*As a reminder, if your child needs any medication(s) during the school day, including all over-the-counter medications, such as Tylenol, Benadryl, and Advil, a medication authorization form must be completed and signed by both a physician AND the parent/guardian. These forms are available in the health office. In addition, the medication must be brought to school in the original container, properly labeled with the student’s name. Plastic baggies are not acceptable.

*No medications will be administered at school without the accurately completed forms. Also, please check the expiration date on medications as we cannot accept expired medications.

Thank you for your cooperation in maintaining a safe environment for our students. If you have any questions or concerns, please don’t hesitate to contact the nurses at the health office.

On behalf of the nurse’s office, we would like to wish everyone a happy and healthy summer!

Dear Parents, Guardians, and Visitors

Due to heightened security requirements backpacks, pocketbooks, or bags are strongly discouraged when you visit the school to pick up a student or meet with the middle school staff. (Please note middle school students are still permitted to bring backpacks, pocketbooks, or bags to school).  Please make every effort to leave everything but your personal identification in your vehicle when visiting the building.  Backpacks, pocketbooks, and bags will be subject to search by security personnel.
Thank you for your assistance.
