Summer School 2019 (Brochure/Registration Form)
2019 SMS Promotion Video

Attention Parents,

Please pick up any unused medication(s) that your child has in the health office by the last day of school. Parents can come in anytime during the school day to pick up medications. The school cannot store medication in the office over the summer and unclaimed medications will be destroyed.

  • As a reminder, if your child needs any medication(s) during the school day, including all over-the-counter medications, such as Tylenol, Benadryl, and Advil, a medication authorization form must be completed and signed by both a physician AND the parent/guardian. These forms are available in the health office. In addition, the medication must be brought to school in the original container, properly labeled with the student’s name. Plastic baggies are not acceptable.
  • No medications will be administered at school without the accurately completed forms. Also, please check the expiration date on medications as we cannot accept expired medications.

Thank you for your cooperation in maintaining a safe environment for our students. If you have any questions or concerns, please don’t hesitate to contact the nurses at the health office.

On behalf of the nurse’s office, we would like to wish everyone a happy and healthy summer!

Any questions regarding OnCourse Connect should be directed to parent.portal@sayrevillek12.net for guardians and student.techsupport@sayrevillek12.net for students.
